The authorized legal representative jointly and severally hereby forever releases, discharges and acquits Absolute Dance Inc. and all their instructors: employees; investors; agents; owners; and directors from any and all claims for damages or injuries of any kind, nature or description. This waiver is to be effective on completion of registration.

Wellness Check & Cohort Agreement *

A parent or guardian must complete the form for all individuals under 18 years of age. If an individual answers yes to any of the questions listed below each time they visit Absolute Dance Inc., they may not enter the facility or participate in any dance activity. Please stay home and use the AHS online assessment tool for further information on determining if testing is required.

Pre-Screening:
Dancers are required to screen each day prior to attending class. Dancers are NOT to attend if you answer yes to any of these questions:

1. In the past 10 days, have you experienced any of the following:
a. Fever
b. New onset of cough or worsening of chronic cough
c. New or worsening shortness of breath or difficulty breathing
d. sore throat
e. Runny nose
2. Do you have any of the following:
a. Chills
b. Painful swallowing
c. Stuffy nose
d. Headache
e. Muscle or joint aches
f. Feeling unwell, fatigue or severe exhaustion
g. Nausea, vomiting, diarrhea or unexplained loss of appetite
h. Loss of sense of smell or taste
i. Conjunctivitis (pink eye)

3. In the past 14 days, did you return from travel outside of Canada, or did you have close contact with someone who is confirmed as having COVID-19?
Should any dancers, family members or current members living in their household have any symptoms of illness, we kindly ask that you do not attend class.

WE THE PARENTS AND/OR GUARDIAN GIVE FULL PERMISSION TO HAVE OUR DANCER PARTICIPATE IN THEIR CLASS COHORT.
I acknowledge and fully agree that entering the facility and/or participation in activities may result in possible exposure to and illness from infectious diseases, including, but not limited to COVID-19.
While rules, personal discipline and following guidelines set out by Alberta Health Services and the Government of Alberta may reduce the risk, the risk of serious illness and death does exist.
I knowingly and freely assume all such risks, both known and unknown, even arising from the negligence of the Releasee or others. I assume full responsibility for participation and exposure and release the facility, its employees, guest choreographers and directors from any liability related thereto.

ADMINISTRATION POLICIES
– Registration fees, first month of classes and theatre fees are non-refundable.
– After Oct 1st, no costume deposits will be returned.
– Choreography fees are non-refundable.
– Competition fees are non-refundable.
– There will be no refunds for missed classes.
– Any outstanding invoices will result in a student not being able to participate in class, and a 10% late fee will apply.
– Any request for a receipt after the original has been provided will be charged a $5.25 administration fee.
– A 4% admin fee will be added to all monthly class fees paid by credit or debit

DANCE CLASS CHANGE / WITHDRAWAL POLICY

– The dance class schedule & instructors are subject to change without notice at any point throughout the dance season.
– Any adjustments made to a student’s schedule must be approved by administration and is subject to class availability.
– No changes can be made after Oct. 1st.
– Any credits must be used within the same season for that same student/family.
– Full Year Withdrawal Policy: A parent/guardian must give notice in writing when withdrawing from a class. Our cancellation policy is one month’s notice. The following month(s) will be returned. No refunds will be issued after December 1st, unless documented medical reason has been provided.
– Sessional Withdrawal Policy: Refunds are not available for any sessional classes. A $53.50 administration fee will be charged for any session of dance classes if withdrawn before the start date. Credit only for any medical documentation provided.
– Summer Camp & Bootcamp/Intensive Withdrawal Policy: No refunds available for Summer Camps. A credit will only be issued if written medical documentation has been provided. The $53.50 admin fee still applies to all credits issued.

COVID-19 REFUND POLICY

– Credit for your programs will be offered only in the event of a government mandated shut down longer than 60 days.
– Credit only, will be available for all 6 week, fall and winter session programming.
– Registration fee, and any costume fees and theatre fees associated with your dancer’s programs are NOT eligible for refund or credit.

MEDIA RELEASE

– I allow Absolute Dance Inc. to use multimedia of my child for advertising purposes on any social media page or studio branding.
