Pre-Primary Acro Jazz (3-4 yrs)

Any students who register for the Winter session of Pre-Primary Acro Jazz will participate in our year end recital and will need to pay the $35 theatre fee and $99.75 costume fee at the time of registration – we have included these fees in our session cost.

Release & Indemnity * 

The authorized legal representative jointly and severally hereby forever releases, discharges and acquits Absolute Dance Inc. and all their instructors: employees; investors; agents; owners; and directors from any and all claims for damages or injuries of any kind, nature or description. This waiver is to be effective on completion of registration.

Wellness Check & Cohort Agreement * 

A parent or guardian must complete the form for all individuals under 18 years of age. All participants must return a form prior to entering the facility.
If an individual answers yes to any of the questions listed below each time they visit Absolute Dance Inc., they may not enter the facility or participate in any dance activity. Please stay home and use the AHS online assessment tool for further information on determining if testing is required.
Dancers are required to screen each day prior to attending class. Dancers are NOT to attend if you answer yes to any of these questions:

1. In the past 10 days, have you experienced any of the following:
a. Fever
b. New onset of cough or worsening of chronic cough
c. New or worsening shortness of breath or difficulty breathing
d. sore throat
e. Runny nose
2. Do you have any of the following:
a. Chills
b. Painful swallowing
c. Stuffy nose
d. Headache
e. Muscle or joint aches
f. Feeling unwell, fatigue or severe exhaustion
g. Nausea, vomiting, diarrhea or unexplained loss of appetite
h. Loss of sense of smell or taste
i. Conjunctivitis (pink eye)

3. In the past 14 days, did you return from travel outside of Canada, or did you have close contact with someone who is confirmed as having COVID-19?
Should any dancers, family members or current members living in their household have any symptoms of illness, we kindly ask that you do not attend class.

I acknowledge and fully agree that entering the facility and/or participation in activities may result in possible exposure to and illness from infectious diseases, including, but not limited to COVID-19.
While rules, personal discipline and following guidelines set out by Alberta Health Services and the Government of Alberta may reduce the risk, the risk of serious illness and death does exist.
I knowingly and freely assume all such risks, both known and unknown, even arising from the negligence of the Releasee or others. I assume full responsibility for participation and exposure and release the facility, its employees, guest choreographers and directors from any liability related thereto.

These guidelines are current for September 2020 and are subject to change without further notice. These guidelines will remain in effect until further notice, as per Absolute Dance Inc. in conjunction with AHS and the Calgary COVID-19:

Studio Policies Acknowledgement * 

• Credit or refund for your programs will be offered in the event of a Government mandated shut down longer than 30 days. Makeup classes will be offered for shut down shorter than 30 days.
• Credit will only be available for all Full Year, Fall, Winter and 6 Week Session programming.
• Registration fee, and any costume fees and theatre fees associated with your dancer’s programs are NOT eligible for refund or credit.

• Registration fees, first month of classes and theatre fees are non-refundable
• After Oct 1st, no costume deposits will be returned
• Competition fees are non-refundable
• There will be no refunds for missed classes
• Any outstanding invoices will result in a student not being able to participate in class, and a 10% late fee will apply

• The dance class schedule & instructors are subject to change with-out notice at any point throughout the dance season.
• Any adjustments made to a student’s schedule must be approved by administration and is subject to class availability. No changes can be made after Oct. 1st.
• Full Year Withdrawal Policy: A parent/guardian must give notice in writing when withdrawing from a class. Our cancellation policy is one month. The following month(s) will be returned. No refunds will be issued after January 1st, unless documented medical reason has been provided.
• Sessional Withdrawal Policy: A $53.50 administration fee will be charged for any Sessional Dance Classes if withdrawing before the start date. No refunds will be given once the session has begun unless written medical documentation has been provided.
• Summer Camp & Intensive Withdrawal Policy: A $53.50 administration fee will be charged for any Summer Camp or Intensives if withdrawing before the start date. No refunds will be given one month prior unless medical documentation has been provided.
• Any credits must be used within the same season for that same student/family.
• I allow Absolute Dance Inc. to use multimedia of my child for advertising purposes on any social media page or studio branding.
